On World patient Safety Day ..17 September .. A reminder on Medicine safety.. Is it deliberate Mistake and or Crime to put fake expiry date on Medicine.? Is it your prescription..right medicind .today my reminder On how the expiry date can be fudged ( pic..on gluco strips sold to me ,.shockingly when i chkd at home the date on sticker affixdd and underneath sticker dhe original date on package from company ... to obscure the expiry date of 11/21 which is about one and half month later..,the retailer printed new sticker(duping ) with new expiry date of 03/22. fixed it over the original company date and batch number....The seller didnt know I jave served as Safety officer for health directorates..i follow ABC check list in my day to day life even having experience as clinical auditor and Quality and Pt safety(QPS) controller for implementation of Healthcare Quality and safety standards :..to continue my servicrs for people I m running a advisory group(..Pt & Family rights plus grievances providing second opinion and advice on malpractice /consumer court complaints). With two decades experience i know all tricks of fraudesters and have been trained how to find them ? . ..i have been inspecting hospitals and healthcare units as surveyor legal action /cancellation of license/fine/imprisonment depending upon negligence amounting to crime Back to story. the man oninquiry (name agency held )wanted to be excused labelling it as stupidity of sales person ..but to me it was drliberate & has criminal intact.When i did RCA..root cause analysis i found it deliberate attempt to xupe a customer. Sadly there is no Q check esp for those working in medical shops or pharmacies.,most of these sales person are not qualified but (10 +2 ) having worked in difderdnt medical shops .In our land besides trafing fake and spurious mdficine 'Medical practice is easiest to be practiced by even non medicos ..Thanks to "google baba " lac of supervision has put pts life to risk ...i have been inspecting all pharma dispensers inquiring from sales man pr woman about "Rights of medicine before dispensing medicine "to pt What global data and Experts claim that -- According to Agency for Healthcare Research and Quality, adverse drug events (ADEs) account for 700,000 emergency department visits and 100,000 hospitalizations Initially five considerations or rights were identified.
